Form 1040A

Form 1040A is a two-page form accompanied by four schedules. It is used by individuals whom do not itemize personal deductions and gross income consists of wages, tips, salaries, interest and dividends, taxable grants and scholarships, IRS and pension distributions, taxable social security, unemployment benefits that total less than $100,000.

Schedules 1, 2 and 3 or Schedule EIC may need to be completed under the following circumstances:

  • More than $1,500 of taxable interest or dividends are reported
  • A credit for child and dependent care expenses is claimed
  • Taxpayer is eligible for the earned income credit
  • Claims a series EE United States savings bond interest exclusion.
